What We Need Before Your Fertility Consultation
In an effort to maximize your fertility consultation please submit the following materials before your appointment:
- Patient Information Sheet
- Female History Form
- Male History Form
- Medical Release Form
- HIPAA Policy
- Release of Information to Spouse or Partner
- Copy of Your Insurance Card and Drivers License
- All Previous Medical Records
